Phius Certified Rater or Verifier Mentorship Letter of Attestation

This letter is an attestation from a Phius Certified professional that they worked directly with a Non-Certified inspector. This is a required piece of documentation when a Non-Certified Inspector is performing the quality assurance on a project, and only applicable when when there is no Phius Certified professional within 200 miles of a project site. See Phius Certification Guidebook Appendix E-2.3.

Phius Certified Rater or Verifier Mentorship Letter of Attestation

Per Certification Guidebook Appendix E-2.3: Non-Certified Inspectors

Where no Phius certified professional is within 200 miles of a project, a project may elect to use a non-Phius certified professional to perform site inspections and testing if approved by Phius Staff.

Requirements are outlined below:

1. Non-certified inspectors shall meet the eligibility requirements for either Phius Certified Rater or Verifier, other than having attended the training.

2. Non-certified inspectors shall contract with and work directly under the supervision of an existing certified Phius Certified Rater or Verifier.
This relationship shall require at a minimum that the Phius Certified certified professional conducts virtual training on the Phius Workbook and testing standards, and reviews all documentation issued by the Phius Certified Verifier/Rater detailing their inspection and testing protocol.

3. The Phius certified professional shall complete a letter of attestation to this training/mentorship. Letter of Attestation can be downloaded below.
